syncscumm: M4 audio design and plan

The design for streaming ScummVM's mixed audio to SyncTERM, and the plan
that implements it.

Recorded here because the design was wrong in a way worth keeping: it
specified the manager's existing stream_chunk() entry point AND arming an
idle notification to catch an underrun, and those contradict -- that entry point never emits one, hardcodes its own channel, and has neither a
cushion nor backlog accounting. It cites syncretro for the approach and
then borrowed a different door's transport. The amendment records the correction and why the extraction is the answer instead.
